Forms to Fill Out
As a new patient to our practice, we need to gather some necessary information about you to provide you with the safest, most appropriate treatment. We’ll need you to fill out a simple form that tells us about general health, dental history, and prescription medications that you take.
For your convenience, visit our forms page and complete your paperwork before you arrive. If you can’t get this done beforehand, please plan to arrive 15 minutes before your scheduled appointment.

Your First Dental Appointment
When it’s time for your appointment, you’ll meet with either Melissa or Tiffany, our dental hygienists. They will spend some time getting to know you and asking about any concerns you have.
The next step is a dental exam to check the condition of your teeth and note any areas of concern on your patient record. Your dental hygienist will then perform a thorough teeth cleaning and check for signs of gum disease.
Setting Up a Treatment Plan
After your cleaning, your hygienist will talk to you about any problems she finds and what treatment you need to correct them. We’ll verify your dental insurance to get an idea of what and how much your benefits cover.
We take all this information into account and develop a treatment plan that we personalize for all your needs, including your budget. Together, we’ll work to resolve any dental problems you’re experiencing and get your oral health back on track.
